After-Hours Nurse Care Advice, Triage Services and Referral Support
During normal clinic hours, your care team is available to speak with you about your health.
After hours, you will be connected to My UnityPoint Nurse. Our Patient Experience Specialists, and our team of Registered Nurses will help you navigate the best next steps. If you do not have a UnityPoint provider, please call our referral hot line, 515-335-7200 supporting all UnityPoint markets at any time, 24 hours a day, 7 days a week, and our team can help connect you to the care you need.
My UnityPoint Nurse
My UnityPoint Nurse provides services supporting assessment and general care advice for patients who have been seen by a UnityPoint Clinic primary care provider in the previous 24 months or have recently discharged from a UnityPoint Health inpatient stay, Emergency Department visit or Urgent Care visit in the previous 30 days. All other individuals, may be directed by our Experience Specialists to seek immediate care or follow up with their primary care provider. My UnityPoint Nurse can also align and recommend a UnityPoint primary care provider to establish care with. That way, the next time you call, you will have full access to our beneficial after-hours nurse triage services.
My UnityPoint Nurse will:
- Serve as an extension of your clinic team after hours
- Assess your symptoms, offer advice and recommend care options
- Provide health information that is important to you
When you call your clinic after hours, you can be connected to My UnityPoint Nurse when:
- You have questions about a health issue
- You are caring for a sick child
- You want to know if you should seek urgent or emergency care for an illness or injury
- You are a caregiver for a family member or friend who receives care from a UnityPoint provider (nurses will need patient consent to share medical information)
