- Guarantor Number: The individual who is financially responsible for the services received. Refer to this number when calling with questions.
- Your Total Balance: This is your total balance for services received at UnityPoint Health.
- Amount Due by: This is the amount of your balance that is due by the indicated date.

Opting in to Paperless Billing
All patients can view and pay bills and statements in MyUnityPoint, our patient portal, anytime, anywhere. Patients can elect to opt in to paperless billing to no longer receive paper billing statements in the mail. Patients who opt in to paperless billing receive text and/or email alerts about new billing statements to the phone number and/or email on file.
To enroll in paperless billing:
- Log in to MyUnityPoint.
- Select “Billing and Payments” on the main drop-down menu.
