It’s powered by “MyChart” to integrate your complete health care record, for fast, easy and secure access to your current health care information.
Manage Your Health Easier from Any Device
With MyUnityPoint, you can manage your health, anytime and anywhere. You can use helpful tools to conveniently manage your health and the health of your family.
From your computer, tablet or smartphone, MyUnityPoint users can:
- Access vaccination records
- Pay copays and bills.
- See test results
- Read messages from your care team
- Review your health history
- Review lab and x-ray results
- Review your personal health history (such as your temperature, pulse, blood pressure, immunizations and providers you’ve seen)
Secure Patient Messaging through MyUnityPoint
One of the best aspects of MyUnityPoint is secure patient messaging.
Secure patient messaging allows you to:
- Request a medicine refill
- Ask a question about your lab results
- Ask a question about your symptoms, treatment plan or medicine
- Request an appointment
- Provide information your care team has asked for (such as blood pressure, weight or blood sugar level)
- Ask any non-urgent questions or concerns
- Provide information or ask questions about your child’s health (with proxy access)
Proxy Access for Children's Health Records
MyUnityPoint can provide access to your children’s health records too. Proxy access allows users to see health information for their children age 11 or younger, and for anyone age 12 and older they have the legal right to make health care decisions for (legal documents are required). Access To MyUnityPoint is Easy
Anyone 18 years of age or older and is a patient of UnityPoint Health can register to use MyUnityPoint. It just takes five minutes to sign up, and your account will start within 1 to 3 days.
You can access a useful web-based version of MyUnityPoint from your mobile device. You can also download a free MyUnityPoint app from the iPhone App Store or Android Market or by searching for MyUnityPoint. After downloading the app, just select MyUnityPoint from the app’s phone book.
