What programs do you have to help me with my bill?
We can assist you in determining eligibility and applying for state programs such as Medicaid or the ACA Insurance marketplace, as well as the UnityPoint Health Financial Assistance program, FINA. Patient Financial Coordinator/Certified Application Counselor, PFC/CAC at (319) 369-7975.
Can I set up payments?
Yes, we do offer payment plan options based on total balance. Please contact our Avadyne payment center at (844) 849-1260 to make payment arrangements.
Why do I need to include financial information and documents for family members or roommates with my assistance application?
Eligibility for the UnityPoint Health financial assistance program, FINA, is based on total household income, which is the same standard used for most State and Federal aid programs.
Why do you need my insurance information for a cost estimate?
Health care billing and patient costs are complex and have many components. In order to provide you with a meaningful patient "out-of-pocket" estimated amount, it is necessary for us to consider any contract we might hold with your insurance company, along with your specific benefits, such as deductible, co-pay, or co-insurance.
How long does it take to get a determination for financial assistance?
This may take up to 3 to 4 weeks.
Do I need to pay before I have my procedure? Where do I pay?
As a courtesy to you we have verified your insurance benefits and we are asking for a down payment based on your estimated patient "out of pocket" cost.
- You may pay by phone by calling our cashiers at (319) 369-7127
- You may pay in person at our hospital cashier.
- You may mail your payment to the following address:

- You may pay when you check into the hospital on the day of your procedure.

Are the baby charges separate from Mom's ?
Yes, baby charges will be billed separate from Mom's.
What is the status of my financial assistance?

Can I come in and talk with someone about my bill and help completing my financial application?
For questions regarding a current bill, please call the number on your statement for assistance.
