To Request a CD of Your Imaging Exam

To request a copy of your imaging exam on a CD for yourself or for your physician, please call Imaging Services at (319) 369-7363. It is possible to send images directly to certain facilities, in which case a CD is not needed.

You will be asked to provide the date of the exam, your name and your birthdate over the phone so your CD can be prepared. You will need to pick up your own images and have a photo ID with you. Imaging Services is located on the 2nd floor of St. Luke's. Enter through the West Entrance and take the A elevators. Upon pickup you will also be asked to sign a Authorization for Release of Medical Information.

Guidelines for Medical Records Request

  • If you require a CD copy of your imaging exam - an authorization must be completed in its entirety. A completed request including signature may be scanned and emailed to our office. 

  • If the patient is a minor (less than 18 years of age), a parent or legal guardian must sign the authorization and complete the parental rights notification.

  • Your request will be processed and we will either mail your copy to the address specified on the authorization form or you may pick up your copy from our offices. Please bring photo identification with you. Our office is located on the second floor of St. Luke's off of the A Ave. Lobby.

  • Requests for hospital medical records should be made through our Medical Records department.

  • Clinic medical records should be directed to the respective clinic.

  • Authorization Forms:
