Recredentialing

Transition Period – March 1, 2021 to September 30, 2023

Over the next 24 months, the UnityPoint Health recredentialing process will transition from our past paper-based, localized process to a centralized, paperless process.

For now, you still must complete any current recredentialing paperwork with your local UPH affiliate(s). You may also need to complete, in tandem, our new online process. The online information will be pre-populated so that the time required for you to complete the online step should take no more than 15 minutes.

The timing of your first necessary recredentialing will start the process.

Please note the following important details:

  • During this transition period, there will be no CVO fees for recredentialing.
  • During this transition period, your recredentialing may be for less than a full 24 months, to ensure that you do not go past the two-year window required by our accrediting bodies. (The CVO will use date reports to assure that no provider will be missed.)
  • New UPH providers will be given a recredentialing date to coincide with the new method immediately after their initial application is processed. Their recredentialing date will be the same regardless of which facility they will practice at, or when they joined that facility.
  • A delegate may assist you in preparing your recredentialing information, but the delegate may not complete disclosure questions, nor sign and submit the recredentialing application on your behalf. You are required to sign into the Provider Portal to review all information, complete the disclosure questions, sign, and submit the application.

Once your reappointment is finalized, you will receive notification from the UPH affiliate's Medical Staff Office and/or PHO/Medimore, as applicable.

There will be additional clarifying instructions provided in individual emails to all providers who begin recredentialing after March 1, 2021.

Centralized Recredentialing

Beginning July 1, 2021, paper-based recredentialing will cease and our new online process will be used exclusively:

After the transition period, all UPH providers will have a permanent recredentialing date, with one cycle for all UPH facilities where the provider is in practice. Recredentialing cycles will begin on April 1 and October 1, depending on the provider's birth month. Once your reappointment is finalized, you will receive notification from the UPH affiliate's Medical Staff Office and/or PHO/Medimore, as applicable.
The following logic will be used to set a provider's permanent recredentialing date:

  • Birthday in January, February, March – recredentialing date will be March 31 of the even year
  • Birthday in April, May, June – recredentialing date will be September 30 of the even year
  • Birthday in July, August, September – recredentialing date will be March 31 of the odd year
  • Birthday in October, November, December – recredentialing date will be September 30 of the odd year

Recredentialing forms will be sent to providers 6 months ahead of time, on this schedule:

  • October 1 for a March 31 recredentialing date
  • April 1 for a September 30 recredentialing date

Remember, a delegate may assist you in preparing your recredentialing information, but the delegate may not complete disclosure questions, nor sign and submit the recredentialing application on your behalf. You are required to sign into the Provider Portal to review all information, complete the disclosure questions, sign, and submit the application.
