Each year, as part of the Global and Professional Direct Contracting (GPDC) Model, the Center for Medicare Services (CMS) measures results across a number of different quality components, encompassing patient experience, care for at-risk patients, preventive care, and patient safety. The results help UnityPoint Accountable Care Participant providers understand how their patients who are aligned to the Global and Professional Direct Contracting Model are doing in key aspects of their care and potential complications, as well as to identify ways they can positively impact a patient's care and quality of life.
