Patient Transfer Center
The UnityPoint Health Patient Transfer Center (PTC) supports our coordinated care approach to ensure patients get the safest, timeliest and best care possible, no matter where they are located.
PTC has a dedicated clinical team available 24/7 to accept patients from inside and outside UnityPoint Health facilities and streamline bed assignments for a smooth transfer process. The PTC team can coordinate:
- Bed availability and bed assignment
- Patient acceptance
- Physician-to-physician communication
- Notification of care team at the receiving facility
Please note: PTC does not coordinate patient transport.
How to Request a Transfer to a UnityPoint Health Facility
To request a transfer to a UnityPoint Health facility, call 833-UPH-XFER or 833-874-9337.
Information Needed for Patient Transfers
To request a transfer to a UnityPoint Health facility, please call our Patient Transfer Center with the following information:
- Patient identifying information (Name, DOB, UPH MRN (if applicable), etc.) Referring location, physician name and contact number
- Reason for transfer request
- Diagnosis
- Level of care needed
- Pertinent clinical information (Vitals, Labs, Airways, Drips, Drains, etc.) to assist with placement
- Preferred destination
