What Does a Partnership with UnityPoint Health Look Like?

Partnering with UnityPoint Health means forming a mutual business agreement where our organization and the partnering organization collaborate in a new way to better serve our patients and communities. Without question, each of our partners has a similar commitment to providing exceptional care.

These mutual business agreements may look and feel different depending on the details agreed upon. Examples of what a partnership might look like include joint ventures, joint operating agreements, affiliations and definitive agreements.

Together, the partnering organizations determine things like the associated name and brand, how to share costs and earnings, and which organization employs the newly integrated staff.

UW Health

The Joint Operating Agreement (JOA) between UnityPoint Health and UW Health formalizes and expands an already significant collaboration in care between UW Health and UnityPoint Health – Meriter. The two organizations have a successful history of working together to provide the best possible care for patients in the Madison community and surrounding areas. Through this partnership, our organizations share best practices, resources, facilities and expertise to meet the growing healthcare needs of south-central Wisconsin and increase care access within our two health systems.

Precedence Utilization Mangement

Since 1992, Precedence, Inc. has conducted UnityPoint Health’s managed care. Precedence Inc. is a nationally accredited, compassionate, cost-effective Utilization Management Program.

HealthSnap

UnityPoint Health is partnering with HealthSnap to offer a chronic care management program to eligible patients. The program aims to bridge a gap and promote better long-term health by providing patients care between office visits, from the comfort and safety of their home. Eligible patients will be contacted via mail and phone with additional information. iOS and Android tip sheets are available for enrolled patients wanting to install the HealthSnap app on their mobile device.
