Community Health Needs Assessment
UnityPoint Health – Meriter is pleased to be a part of the Healthy Dane Collaborative's Community Health Needs Assessment. As part of health care reform, all not-for-profit hospitals are required to conduct a community health needs assessment on a triennial basis, to determine the health needs of the community. To do this in Dane County, Meriter partnered with SSM Health/St. Mary's Hospital, UW Health, Stoughton Hospital, Group Health Cooperative of South-Central Wisconsin, Public Health Madison, and Dane County to create this assessment. This collaborative demonstrates the core belief of all our organizations – that the health of our community is paramount. Nothing is greater than helping our neighbors stay and be well.
For this process, the Healthy Dane Collaborative utilized publicly available data, local Public Health Madison and Dane County data, and held sessions with community members. We are proud to present one shared CHNA to our community.
Through this work, we have a much better understanding of our community's health and the opportunity for even better health. The CHNA also guides our priorities for community health improvement. Of course, we know that this Community Health Needs Assessment is not perfect, nor does it reflect each person's experience with health. It does, however; provide us with a snapshot of our community and provides insights into ways we can help make Dane County healthier.
