Position Summary: Penn Center Community Based Nurse utilizes the nursing process to collaborate within a team approach, utilize evidence-based practice to collaborate within community- based team to support adults with mental health needs in their own homes. Consults, advises, and provides when necessary, healthcare needs and services. Performs all essential functions of care coordination. Under the supervision of the Clinical Nurse Manager, coordinates daily support services or skill development as needed by the individuals. Enhances clients understanding of their own medical needs.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
Assists and advises individuals who have requested assistance or who have been identified as needing assistance with healthcare and related services.
Assists Clinical Nurse Manager and Coordinators with supervising and coordinating daily support services as needed for individual needs.
Provides direct nursing care services and daily support services with the approval of the clinical team.
Maintains positive, professional working relationships with community members of treatment team.
Transports and accompanies individuals to medical appointments when health advocacy is needed, or supports others in doing so, especially at times of health transitions, surgery, etc.
Participates in total client care through the development, planning, implementation, and evaluation of each client’s Individual Service Plan; including the physical, psycho/social, mental health, legal and spiritual needs of the client.

Attendance is an essential part of this position.
Provides and/or assists with education of individuals and wellness topics.
May occasionally be requested to be available for consultation on off hours when other clinical care is not available.
Other related duties as needed and/or directed.

QUALIFICATIONS:
Education: Graduation from an accredited LPN or RN nursing program.
Experience: LPN or RN with desirable to have a minimum of two years experience with an emphasis in human services/mental health/mental retardation, and/or gerontology.
Skills, Knowledge and Abilities:
Must be supportive and carry out work activities and responsibilities aligning with the mission of Penn Center, Inc.
Must be able to respect and maintain confidentiality of sensitive information.
