UnityPoint Health Credentials Verification Office (CVO)
The credentialing team at the CVO is responsible for processing providers' initial and recredentialing applications and updating provider demographics in the credentialing database.
The CVO shares information with the Physician Hospital Organization (PHO)/Medimore, Inc. to assist with payer enrollment and network management, and to the medical staff offices (MSOs) to assist with privileging providers for UnityPoint Health facilities. Our goal is to supply provider-centric credentials verification that is electronic, secure and efficient.
Initial Credentialing Application

In your request, you may include the email address of a delegate person. This delegate may complete the demographic, malpractice, licensure, schooling and other informational portions of an initial application or recredentialing application. Delegates may NOT complete disclosure questions or the delineation of privileges form (if applicable), nor sign and submit an application on a provider's behalf. Providers are required to sign into the Provider Portal to review all information, complete the disclosure questions and the delineation of privileges forms (if applicable), sign and submit the application.
Once the application request is processed (in approximately 5-7 days), a welcome email will be sent to the provider and the delegate with a link to our online portal application. If you have any difficulties accessing the portal, please refer to our Initial Credentialing Online Portal Application Tip Sheet or contact the CVO.

Transition Period – March 1, 2021 to September 30, 2023
Over a 24-month period, the UnityPoint Health recredentialing process is transitioning from our past paper-based, localized process to a centralized, paperless process. Online information will be pre-populated so it should take you no more than 30 minutes to complete your request.
Please note the following important details during this transition period:
- There are no CVO fees for recredentialing.
- Your recredentialing may be for less than a full 24 months, to ensure that you do not go past the two-year window required by our accrediting bodies. The CVO uses date reports to assure that no provider is missed.
- New UPH providers will be given a recredentialing date to coincide with the new method immediately after their initial application is processed. Their recredentialing date will be the same regardless of which facility they will practice at, or when they joined that facility.
- A delegate may assist you in preparing your recredentialing information, but the delegate may not complete disclosure questions, the delineation of privileges form, nor sign and submit the recredentialing application on your behalf. You are required to sign into the Provider Portal to review all information, complete the disclosure questions, sign and submit the application.
Once your reappointment is finalized, you will receive notification from the UPH affiliate's Medical Staff Office and/or PHO/Medimore, as applicable. There will be additional clarifying instructions provided in individual emails to all recredentialing providers.
After September 30, 2023, all UPH providers will have a permanent recredentialing date, with one cycle for all UPH facilities where the provider is in practice. Moving forward, recredentialing cycles will be dependent on your birth month and year. You will receive notification of online recredentialing 6 months in advance of the recredentialing date.
Remember, a delegate may assist you in preparing your recredentialing information, but the delegate may not complete disclosure questions, the delineation of privileges form, nor sign and submit the recredentialing application on your behalf. You are required to sign into the Provider Portal to review all information, complete the disclosure questions, sign, and submit the application.
Credentialing Application Tracker 2.0
The Credentialing Application Tracker (CAT) for initial applications and recredentialing applications is on the CVO's ServiceNow home page. Simply enter a provider's NPI for a high-level view of where they are in the credentialing, privileging and payer enrollment process. The CAT provides dates and the percent of task complete. The latest CAT 2.0 tool provides more information regarding the status of your references, hospital information and other aspects of the process.

Change of Information
Use the ServiceNow ticketing system if you need to change or correct your current information in the credentialing database (name, home address, practice location, request additional privileges, etc.). Please be aware that the CVO does not manage any updates or access to HR/Employment, EPIC, or any other databases beyond the credentialing system and online credentialing application portals.
Resignation of Hospital Privileges and/or Medimore PHO Participation
Use the ServiceNow ticketing system if you will be resigning your hospital privileges or enrollment in Medimore PHO.

Use the CAT self-service tool located within our ServiceNow website to quickly check the status of your application online. You may also send questions to UPH_CVO@unitypoint.org.
