The staff of Allen Occupational Health Services is comprised of highly skilled professionals in the practice of Occupational Medicine. Five physicians, and two advanced registered nurse practitioners, a certified occupational health registered nurse, five registered nurses, three licensed practical nurses, and two medical assistants, are the essence of the clinical healthcare team. The organization is further supported by fully trained and qualified office and business personnel for registration, scheduling, and customer service responsibilities.

About Our Medical Providers

Jonathan Fields, MD is certified by the American Boards of Occupational and Environmental Medicine. He is a certified Medical Review Officer. Dr. Fields received his training from the University of Iowa Hospitals and Clinics and has been practicing Occupational and Environmental medicine since 2013. Dr. Fields comes from the industry with an engineering background. His clinical interests include sports medicine and worksite injury prevention. When he isn't in the clinic you can find him biking, hiking, rock climbing or playing outdoors with his wife and children.

Nazee Jabbari, MD is certified by the American Boards of Occupational and Environmental Medicine. She is a certified Medical Review Officer. Dr. Jabbari completed her residency with the University of Iowa Hospitals and Clinics and has been practicing Occupational and Environmental Medicine since 2011. When not in the clinic Dr. Jabbari enjoys spending time with her husband and two kids as well as trying all the latest fitness trends.


Jack Luke, MD comes to Allen with over 17 years of experience as an Occupational Health Medical Director for a heavy construction and agricultural equipment manufacturer. Dr. Luke earned his medical degree from the University of Iowa, College of Medicine, in 1991. He completed his internship at Eisenhower Army Medical Center in 1992. During his tenure as an Occupational Health Medical Director, Dr. Luke provided oversight and management of two high volume occupational health clinics. In addition, he was actively involved in writing hearing conservation programs, developing on-line databanks of work activity restrictions and leading an Integrated Ergonomics Committee. Dr. Luke was also instrumental in the development of an on-site fitness center and production floor based athletic training program.

Kenneth McMains, MD is an Adjunct Associate Professor in the College of Public Health at the University of Iowa and an Adjunct Assistant Professor at Des Moines University. He is trained in the area of Independent Medical Evaluations (IME) and Impairment Ratings and has been performing these evaluations for more than 20 years. He is also a certified Medical Review Officer (MRO) and has been so for more than 15 years. Dr. McMains' past experience as an emergency services physician, family practitioner for rural communities, diabetes educator, and mental health director complement his expertise in assisting businesses with their most valuable assets - their employees. His dedication to occupational and environmental safety and health is evident in his involvement and commitment as the Chairman of the Residency Advisory Committee for Occupational Medicine Residents at the University of Iowa Hospitals and Clinics. Dr. McMains also provides clinical training in occupational medicine for residents in the Black Hawk Family Practice Residency Program and the University of Iowa occupational medicine residents.

Steve Olsen, DO is a graduate of the Occupational Medicine Residency Program at The University of Iowa Hospitals and Clinics. He is certified as an independent medical examiner (IME) and Medical Review Officer (MRO). Dr. Olsen's extensive thirty years of military experience and a twenty year business background provide him with an insight into the unique medical needs of employees and employers. He is a member of the University of Iowa Residency Advisory Committee and assists in clinical training of physicians in the residency program.

Jean Osgood and Heather Cue  are board certified Advanced Register Nurse Practitioners.
