The Helping Educate and Link the Homeless (HEALTH) program serves as a bridge for those individuals who are uninsured, unemployed or disconnected from medical and other community services.
It provides a direct case management program for homeless patients with significant health care needs, and links patients with primary care services and community resources, provides education about medical conditions, respite care following an illness or surgery and coordinates follow up patient care plans. The HEALTH Program offers these services at locations throughout our community, adhering to the street medicine model that has always been the core of the program.
Gifts to support the HEALTH program play a vital role in providing medical care, educational materials and respite care for those recovering from illness and surgery in our community. With donor support in 2018, the HEALTH program provided 246 days of respite care for individuals who needed extra time to recover following surgery or an illness and directly assisted 333 homeless individuals in our community.
