Free parking is available in the ramp behind the building. There is covered access to the second floor. Once inside the building, look for signs to UnityPoint Health - Meriter - Wellness Center.

We offer a variety of patient care and wellness programming to meet your needs. Our team of Clinical Exercise Physiologists, Fitness Specialists, Registered Nurse, Respiratory Therapist and Dietitian is here to assist you.

Specialty Patient Care Services

Cardiac Rehabilitation

The Outpatient Cardiac Rehab program builds upon the skills and life changes you began working on while in the hospital. We are AACVPR Certified. Our program includes:

  • Individualized plan to meet your long-term health goals and personal exercise needs
  • Risk factor reduction to include:
    • Comprehensive nutrition assessment and personal nutrition plan
    • Cholesterol/Blood Pressure and Diabetes Management
    • Progressive Exercise
    • Weight Management Strategies
    • Smoking Cessation
    • Stress Management

Pulmonary Rehabilitation

Pulmonary Rehabilitation is an outpatient program of exercise and education for those with lung disease or conditions. While we cannot cure your disease, it is our goal to introduce you to ways to help improve your daily life. We are AACVPR certified.

Program Objectives:

  • Decrease breathlessness
  • Increase strength
  • Increase stamina
  • Improve feeling of well-being
  • Improve nutritional status
  • Learn to use oxygen and inhalers appropriately
  • Improve knowledge and understanding of your disease and treatments

Supervised Exercise Therapy for Peripheral Artery Disease (PAD Rehab)

Supervised Exercise Therapy (SET) is an exercise program for people who have leg pain with walking (called intermittent claudication) which is caused by peripheral artery disease (PAD). Research shows that SET is an excellent first-line treatment for PAD.

Expected Benefits:

  • Improvement in how far and how long you can walk before having leg pain
  • Increase in activity levels and improvement in quality of life
  • Reduced pain/discomfort in your leg(s) when you walk
Registered Dietitian

Medical Nutrition Therapy that focuses on diet and lifestyle changes that can prevent or reduce your health risks. Instruction and counseling regarding a heart-healthy diet, weight loss, lowering cholesterol, BP or salt intake, basic healthy eating, meal planning and mindful eating.

Fee-for-Service Programs

Stay Fit
An independent supervised exercise programs for people with chronic medical conditions or those requiring assistance in developing an exercise program to improve their health. This program involves a client consult with development of an exercise prescription that includes education about exercise progression and risk factor modification.
Exercise Consults

This program is designed to provide a personalized exercise guidelines including exercise demonstration and progression based on your individual goals. Gain confidence in your ability to exercise safely before transitioning to a community-based exercise facility and/or a home-based workout program.

  • Supervised exercise with a certified personal trainer.
  • Increased knowledge and understanding of exercise safety and progression.
  • Increased confidence in abilities.
  • Increased support for developing healthy lifestyle behaviors and enhancing prevention of chronic disease risks.
  • This program can be for any fitness level.

Beginning Exercise Training Program

Provides a personalized, exercise training programs to include exercise demonstration, progression, education. Work with a Personal Trainer over a series of four to eight sessions in order to gain confidence in your ability to exercise safely following recommended exercise guidelines for health and fitness.
