Diabetes and Nutrition Education Clinic

Our care team is devoted to helping individuals on their personal wellness journeys. We offer information, instruction and support through educational programs and counseling sessions.

Talk with your doctor about scheduling an appointment with one of our clinic providers. A provider referral is required for a Diabetes and Nutrition Education Clinic consult.

Diabetes Education

Our diabetic educator’s role is a true partnership with primary care providers; her goal is to improve each person’s quality of life as they live with diabetes. She supports patients with education and self-management skills. She is certified through the American Diabetes Association’s (ADA) Education Recognition Program (ERP).

Our registered dietitians have the experience and expertise to help you build a healthy lifestyle.
