A Program of All-Inclusive Care for the Elderly
A unique feature and key to the success of the program is the PACE center. Located in Sioux City, Iowa, the center provides an opportunity for participants to build relationships with peers in the recreation center, receive medical care in our clinic, enjoy a hot and nutritious meal in our dining area and participate in rehabilitation and therapy support all in one location. The center also provides transportation for the participants to and from their home to the center, or other necessary appointments that they may have in the community This center separates PACE from an accountable care organization (ACO) or managed care organization (MCO) because participants can receive all of their necessary care under one roof, eliminating the need for multiple trips and multiple visits with various doctors.
You've spent a lifetime building your home and the wonderful memories within it. Whether you're searching for assisted living, nursing home or senior care, our team at Siouxland PACE will work together with you and your family to keep you safe and comfortable in your home for as long as possible, while offering a level a care that will provide peace of mind.
