We are committed to caring for our participants during this difficult and uncertain time. Siouxland PACE is exploring all opportunities to continue providing excellent care for all our participants, including the option for telehealth visits. We will continue to keep participants and families up to date with any new developments related to PACE services. If you have a scheduled appointment, please plan to keep this appointment. PACE recommends only going out for your medical appointments.
If you are experiencing any new or worsening health symptoms such as a fever, cough, or shortness of breath, please contact PACE at 712-224-7223 to speak with your team, this phone is answered outside of business hours as well.

- Stay home if possible, keep away from people.
- When you are around other people stay at least 6 feet from them.
- Wash your hands OFTEN and for at least 20 seconds.
- Stock up on supplies (with someone's help), so that you don't have to go to the store. PACE may be able to help you if you need any supplies.
- Call PACE if you have any concerns.
The Siouxland PACE Team wants all our participants and their caregivers to know that we understand this is a challenging time. We want you to call us for any reason, we are here for your throughout this entire COVID-19 emergency. We will get through this together, while staying apart!!

Siouxland PACE has access to a variety of programs to help with your health care needs. From home health to rehabilitation, our PACE team can coordinate a full-range of resources for you including primary and specialty medical care, medications, medical transportation and therapy. A Program of All-Inclusive Care for the Elderly
A unique feature and key to the success of the program is the PACE center. Located in Sioux City, Iowa, the center provides an opportunity for participants to build relationships with peers in the recreation center, receive medical care in our clinic, enjoy a hot and nutritious meal in our dining area and participate in rehabilitation and therapy support all in one location. The center also provides transportation for the participants to and from their home to the center, or other necessary appointments that they may have in the community This center separates PACE from an accountable care organization (ACO) or managed care organization (MCO) because participants can receive all of their necessary care under one roof, eliminating the need for multiple trips and multiple visits with various doctors.
