UnityPoint at Home Named to 2015 HomeCare Elite
UnityPoint at Home announced that five of its locations – Des Moines, Fort Dodge, Sioux City, Storm Lake and Waterloo – have been named to the 2015 HomeCare Elite™, a compilation of the top-performing home health care providers in the United States.
Now in its tenth year, HomeCare Elite identifies the top 25 percent of agencies based on performance measures in quality outcomes, process of care measures, and financial performance.
The award is sponsored by OCS HomeCare by National Research Corporation, the leading provider of home health and hospice metrics and analytics, and DecisionHealth, publisher of the most respected independent publication in the home care profession, Home Health Line.
“The 2015 HomeCare Elite winners deserve credit for demonstrating high-quality care in their communities. As quality care initiatives become top-of-mind for healthcare providers, we recognize this as a remarkable achievement and are pleased to make this our 10th year of acknowledgement for top performing home health organizations,” said Christine Lang, VP of Product Development for OCS HomeCare. “We congratulate UnityPoint at Home on being one of the top home care agencies in the country.”
Todd Richard, vice president of operations for UnityPoint at Home, credits compassionate, knowledgeable staff and the organization’s ongoing commitment to innovation and care coordination with its ability to rank amongst the top home health care providers in the country.
“We are very proud of our UnityPoint at Home locations on the HomeCare Elite list. Each one has demonstrated our mission of improving the health of the communities we serve, and helping our patients achieve and maintain their best level of well-being and independence. Our first priority is always meeting the needs of each and every patient, and we strive to do this through communication, innovation, and coordinated care,” Richard states. “This collaboration ensures that every one of our professionals involved in a patient’s treatment is familiar with the medical history, current level of health, and future goals.”
UnityPoint at Home implements innovative clinical strategies that improve patient care, decrease unnecessary hospitalizations and reduce costs. Clinicians are trained in integrated care management, with the knowledge and tools necessary to deliver interventional care in the home to patients with conditions such as heart disease, chronic obstructive pulmonary disease (COPD), diabetes and other chronic illnesses. Working collaboratively with patients and families, clinicians provide education and encourage self-management techniques, while coordinating care with physicians.
“At the end of the day, it’s about being proactive and not reactive with care,” states Richard. “Our clinicians are focused on making sure they are the direct link to our patients’ primary care providers. Good communication and connections result in swifter interventions, higher quality care and positive patient experiences and satisfaction. This is reflected in UnityPoint at Home being ranked amongst the top home health care providers in the United States.”
