Patient Tools
We’re upgrading to MyUnityPoint. Starting March 29, MyUnityPoint is your patient portal for managing appointments, viewing records and communicating with your care team.
Access Your Records From Before March 29
Your historical records from our previous system are still available through the portals below. We recommend saving or downloading any records you need, as these portals will be decommissioned in the future.
Once the legacy portals are decommissioned, call (877) 448-1721 to request access to historical records.
