St. Luke's Transitional Care Center (TCC) serves patients in Cedar Rapids and the surrounding area. This is where you go to heal following an injury, surgery, or serious illness. TCC is unique to the Cedar Rapids community—designed to bridge the distance between a hospital setting and your long-term home.

In addition to short-term care, TCC offers long-term care and enhanced options. Enhanced care allows individuals extra time before transitioning home after a skilled stay. Patient Daily Schedule

Patients and their loved ones will have access to their schedule through a web application (or paper, if preferred), from wherever they are. This scheduling solution is powered by Skilled Day.

Individualized Care
Patient care is driven by input from hospital doctors and the patient’s primary doctor. From there, our team of providers, nurses, social workers, physical, occupational and speech therapists design a personalized care plan. From regular visits by a nurse practitioner to a custom rehabilitation program geared at strengthening the body, all services are uniquely tailored to help each patient achieve their individual health goals.

Coordinated Approach

Each patient’s team meets at their bedside early in your stay, and every week during their recovery. We discuss how they’re doing and identify goals to work on to get them back home. With medical providers onsite five days per week, the team includes:

Your Own Space

Each resident enjoys their own private space in our skilled nursing facility, creating a peaceful and personalized environment where they can relax and feel at ease.

All rooms include private bathrooms. The bathrooms feature zero-entry showers with built-in bench seating, offering both safety and comfort while promoting independence.

Every room comes fully furnished and includes a television with cable service and Wi-Fi access, making it easy for residents to stay entertained and connected with family and friends. For enhanced comfort, residents rest in electric hospital beds that are larger than standard twin-size beds, providing extra space and customizable positioning for the best support.

Both short-term care patients and long-term care residents will benefit from large open hallways with several welcoming social areas to mingle and visit with others. We also have a variety of outside sitting areas to enjoy warm sunny days.

Our transitional care center blends safety, functionality, and comfort so each resident feels secure, respected, and truly at home.
