We Want to Hear From You: Community Health Assessment Survey

(GRINNELL, IA – June 29, 2022) UnityPoint Health® - Grinnell is conducting a Community Health Assessment (CHA) survey. This study is completed every three years to identify major health problems, gaps in services and other factors which may contribute to less-than-optimal health status for residents.
Questions in this survey include topics about transportation, health, food availability and more. There are two ways Poweshiek County residents can participate:

An online survey. The survey is available at prcsurvey.com/begin/Grinnell2022CHNA and closes Sunday, August 21, 2022.

Phone interview starting July 2022. Phone calls will be conducted by Professional Research Consultants team members on behalf of UnityPoint Health – Grinnell. Individuals living in Poweshiek County are selected at random for these phone calls.
Participants are encouraged to only complete one form of the CHA with either the online survey or participating in the phone call.
“It is important that we gather as many surveys as possible,” says Shauna Callaway, director of public health. “Once this assessment is complete, we will work to develop strategies and goals for improving the health of our community based on the feedback we receive. We encourage individuals that are called to participate in this survey.”
