About Us

Trinity Diabetes Center/Healthy Living provides education and services to individuals with, or at risk for diabetes, and those looking to live a healthier lifestyle. Diabetes education helps individuals gain the skills and knowledge needed to change their behavior and successfully manage diabetes and its related conditions. Our diabetes educators will partner with you, your support caregiver and your provider to optimize your diabetes control while in our care. We will help teach you the skills needed to live successfully with diabetes.

Your diabetes care team includes your doctor, nurse, care coordinator, social worker, dietitian and pharmacist. Your care team may also include certified diabetes care and education specialists (CDCES); these team members have experience educating people with diabetes and have passed an examination to become certified. They work with you as your teacher, coach and counselor. To meet your needs, diabetes education is provided through group classes or individual sessions.

Diabetes Support and Resources

National Diabetes Prevention Program

This year-long program begins with weekly meetings for the first four months, followed by meetings every 2-3 weeks for the rest of the year. By taking this program, you will receive the skills to lose weight, become more active and manage stress. You will work with trained lifestyle coaches to help you reach your goals, and receive encouragement and friendly competition from peers.

You may be at-risk for prediabetes if you are over 45 years of age, overweight, have a family history of diabetes, and exercise less than three times per week.

Requirements to Participate: 
  • Not diagnosed with diabetes
  • Have at least one of the following:
    • Diagnosis of prediabetes within the past year
    • History of diabetes while pregnant
    • At risk for prediabetes

Stroke and Vascular Checks

Trinity Healthy Living and Trinity Cardiovascular Lab partner together to offer four different screening options for individuals. Vascular Surgeon, Dr. Michael Willerth, UnityPoint Clinic Surgery – Fort Dodge, reviews all vascular screening results and recommends follow-up care, if needed. Screening results are sent directly to the participant.

The cost for all four screens is $115. No Medicare, Medicaid or private insurance accepted.

Available Screenings

Stroke Screen

The stroke screen is for individuals who may be at risk for a stroke. Risk factors include family history of strokes, high blood pressure, smoking, heart disease, obesity, high cholesterol, heavy use of alcohol and lack of physical activity. Cost is $35.

The screening includes:

  • Risk factor questionnaire
  • Blood pressure and pulse check
  • Non-fasting cholesterol determination
  • Heart rhythm with 3-lead EKG for heart irregularity
  • Ultrasound of the carotid arteries
Abdominal Aortic Aneurysm (AAA) Screen

The abdominal aortic aneurysm (AAA) screen is for individuals at risk for the development of an AAA. The greatest risk factor is a family history of AAA. Other factors include age (40 and older), male gender, tobacco use, high blood pressure and peripheral vascular disease. Cost is $35.

Peripheral Vascular Disease (PVD) Screen

The peripheral vascular disease (PVD) screen is for individuals at risk for the development of PVD. Candidates are individuals with diabetes, cardiovascular disease, smokers and those who experience difficulty walking or leg pain. Cost is $35.

Osteoporosis Screen
The osteoporosis screen is for individuals interested in learning if they are at risk of developing osteoporosis. Cost is $25.

Public Walking Path

As your partner in health, we have mapped out a walking path inside Trinity Regional Medical Center for individuals to walk in a safe, climate controlled space. We ask all walkers to take a brief orientation to learn the route and get a walking tag before starting. Call (515) 574-6350 to become a hall walker.
