Our goal is to improve the quality of life for patients with chronic conditions such as diabetes, heart disease or lung disease by providing them with the proper education, mentoring and coaching to better understand their diseases and learn how to lead healthy and productive lives.
Medication Management
It is important to take medications in the right dosage at the right time, especially when taking many different kinds to manage an array of chronic conditions. If medication is not taken correctly, it can lead to other complications. Our nurses educate patients about their medications and how to take them while also monitoring the different kinds being taken and their effects.
Wound Care
Successful wound care is a vital part of healing and allows a patient to have a lifestyle as full and productive as possible. Our wound care experts are registered nurses certified by the Wound, Ostomy and Continence Nurses Society (WOCN), the leading national accreditation society for this type of specialized nursing. They work with patients to provide educational support to help prevent future issues, and utilize the latest wound management resources. Some of the most common conditions for wound and ostomy care are ulcers, diabetic wounds, and surgical wounds.
Infusion Care
Our licensed infusion staff provides a wide variety of nursing care and pharmacy support for IV medications including antibiotics, pain management, chemotherapy, nutritional supplements, hydration and other specialized therapies. Home infusion therapies are prepared under the supervision of specialized pharmacists and are delivered and administered to patients by infusion nurses or trained caregivers.

Patient education focuses on helping patients understand their conditions and how to manage their symptoms. Health care professionals work with patients to help develop a plan for preventative measures that will allow a healthier and more fulfilling life.
Care Coordination
This service is an approach to health care that focuses on providing quality care for patients by assigning a nurse as a primary point of contact to coordinate care across all settings. The nurse is responsible for making sure the patient is receiving the appropriate type of care, to find the resources that are needed, and to be an advocate for the patient. The nurse works closely with the patient and family on a regular basis and is always available to answer questions.
