How long have you been a part of UnityPoint Health - Finley Hospital?

I have been a member of Finley Hospital’s team since June of 2012.

Describe your role at Finley Hospital.

As a Nurse Case Manager, my role centers on the care plans of patients throughout their stay at Finley Hospital. I make sure that there is a fine balance between the care they need and what is needed from the insurance side of their health care needs. I work in conjunction with nursing staff and providers to make sure that the patient’s needs are met in terms of appropriate testing and how we can work together to improve efficiency of patient stay.

What does a typical day look like for you?

Typically, my day starts with patients receiving care on Finley Hospital’s 4th floor. I meet with patients directly, and talk with them about their care plans. I also round with various providers on the floor and touch base with them in terms of patient care plans. In addition to this, I lead care conferences with nursing staff, and work with them to ensure that each of our patients receive the Best Outcome Every Patient, Every Time.

If someone was interested in case management work in a health care facility, what would you tell them about Finley, and why it’s a great place to do this type of work?

UnityPoint Health – Finley Hospital is a great team to be a part of. We’re an organization in the Dubuque and Tri-State community that is consistently growing and developing new ways to provide quality coordinated care to our patients and their families. I’m part of an organization that makes an effort to own every moment that has the potential to provide best outcomes for our patients.

In what ways does your role have an impact on our community?

The case management team at Finley Hospital has just as strong of an impact on community, as it does directly with our patients. We work with our community in skilled nursing facilities, provide follow-up phone calls with our patients at home, and even provide pre-operative education opportunities – such as our Orthopedic Nurse Navigator program – for members of our community.

What does Best Outcome Every Patient, Every Time mean to you?

To me providing the Best Outcome Every Patient, Every Time means that we provide an environment for our patients that doesn’t just provide quality, coordinated care; but one where patients and their families feel safe and not judged. Our patients are at the center of our care, and we remain strong in that promise.

Describe your favorite memory while working at Finley Hospital.

Many memories come to mind when thinking back to how my role has had an effect on our patients and community. One that stands out to me in particular was when I first began my work as a Nurse Case Manager at Finley Hospital. The Case Management team was so inviting, and it felt good to become part of a family that had the same goals in mind when providing the best care for our patients.
