Community Based Recovery Services

Community Based Recovery Services (CBRS) provides a continuum of mental health treatment, supports and habilitation services designed to assist adults with a serious mental illness to overcome barriers and develop a happy and meaningful life. CBRS staff are committed to helping individuals who desire increased satisfaction in life areas including home, work, school and relationships. Services provided:

First Episode Recovery Support Team (FERST)

FERST helps individuals navigate the road to recovery from a first episode of psychosis. FERST is a multidisciplinary team that includes Team Director, Psychiatrist, Individual Resilience Therapist, Family Education Program, Supported Employee and Education, and Community Support Specialist. With a focus on early intervention, illness management, shared decision making, active learning, and recovery clients who participate in FERST report improved outcomes in relationships, work, employment, and overall life satisfaction.

Abbe Center's FERST program is one of three new mental health block grant funded programs in the state of Iowa that based upon the NAVIGATE treatment model.

Program for Assertive Community Treatment (PACT)

PACT services are high-intensity community mental health services provided in the Cedar Rapids area by a multi-disciplinary team. This team includes a psychiatrist, physician's assistant, psychiatric nurses, master's level clinicians, case managers, and a vocational and substance abuse specialist. All services are provided by and coordinated by the team. Daily team meetings allow for close monitoring and timely interventions to meet changes in the needs of those they serve. PACT offers a daily medication monitoring, medication management, and assistance with daily living skills, housing and employment. PACT is intended to provide services to individuals with serious mental illness—typically schizophrenia or a similar illness that includes psychosis. Individuals served in PACT often decrease reliance on emergency services and institutional placement as access to staff is offered 24 hours per day and treatment focus is on recovery.

Supported Community Living (SCL)

SCL services are moderate-intensity community mental health services designed to be teaching, supportive and transitional in nature. Treatment focus is on the development of independent living, interpersonal and symptom management skills. SCL staff offices are located at on-site living complexes in Cedar Rapids. Individuals in this program are encouraged to take an active role in their goal setting and recovery efforts. Individualized treatment plans are created with clients and their natural supports as desired by the client.

Community Support Services (CSS)

CSS provide low-intensity community mental health services designed to assist individuals with a serious mental illness to live and work in the community. Services are individualized to match needs and may include assistance with housing, applying for benefits/entitlements, illness education, employment, assistance with ADL's, etc. Services are provided in the home, office or community settings. Groups are periodically offered to meet specific treatment needs.

Wellness and Recovery Services

Clinical staff and Peer Support Specialists partner in the provision of recovery-oriented services including:

Peer led Services

Club 520 (Cedar Rapids) and Together Not Alone (Iowa City)
Peer support specialists are responsible for the planning and implementation of services including: support groups, co-occurring recovery groups and vocational groups.

Community Based Peer Support Services
Services provided by peer support specialists in the home or community that promote recovery and community integration.

Iowa Warm Line
The Warm Line is a peer-run, telephone based, non-crisis and confidential listening line for anyone struggling with mental health or substance abuse issues. The Warm Line is staffed by individuals who have been through a similar journey and are in recovery themselves.

Intensive Psychiatric Rehabilitation (IPR)

Curriculum-based service that allows individuals to identify and overcome barriers and assist them in learning the decision-making process. IPR is designed for individuals who desire to make a change in their life related to a living situation, work or education.

Projects for Assistance in Transition from Homelessness (PATH)

PATH services include community-based outreach and case management for people with serious mental illness who are experiencing homelessness or at risk of becoming homeless. 

Available in: Linn County, Johnson County

Life Skills

Skill-teaching habilitation service that focuses on providing structured daytime services to individuals with serious mental illness.

Funding options: Community Based Peer Support, IPR and Life skills may be funded by Managed Care Medicaid, MHDS Region, or private pay. All other Wellness and Recovery Services do not require individual funding sources.
