Medical Records

You can view and download medical records by accessing your MyCarle online portal. MyCarle gives you the ability to view and download lab and radiology results, health history including allergies and immunizations, after visit summaries and most hospital and outpatient provider notes. To access medical records that are not available on your MyCarle portal, please submit a request to Health Information Management.

There are two ways to request medical records: You can use the Online Records Request Tool or submit a signed hard copy of a Release Authorization Form.

Our online records request tool verifies your identity by asking for a photo of your driver's license, which can be taken via webcam or smartphone.

  • Records are retained for a minimum of 10 years, in some cases longer.
  • All Birth/Death Certificates must be requested through the County Courthouse. http://www.peoriacounty.org/countyclerk/ 
  • There may be minimal costs associated with certain types of requests.

Very Important
Authorizations will expire one year from the date signed unless otherwise specified. Mental health records may require special authorizations, signatures or releases. Please call the Health Information Department at (217) 902-6500 if you have questions about completing the forms or obtaining copies of your medical records.

Proxy MyCarle Access: Parents and Legal guardians may request MyCarle access to their children's accounts by completing the MyCarle Proxy Access Form online or obtaining a copy of the form at your next outpatient provider visit.
