In 2019 UnityPoint Health®, Human Service Center and Tazwood Center for Wellness came together to form UnityPoint Health- UnityPlace, a new non-profit organization dedicated to meeting the growing addiction recovery and behavioral health care needs of the community.
The new organization will work to enhance and improve community access to a fully- coordinated regional network of behavioral health care providers, specialists and substance abuse and rehabilitation services. UnityPoint Health - UnityPlace also hopes to attract and retain talented and professional providers of these services, expand services, and improve patient outcomes.

Taking the first step toward healing and health is important. With outpatient, inpatient and residential placement programs available, we are here to support you. Connect with our team to discuss your concerns. An assessment appointment will be scheduled at the best location of service for your needs. (Phone screenings and phone assessments are offered at most locations.) A recommendation for care will then be discussed with you.
UnityPoint Health-Unity Place and its subsidiaries, the Human Service Center and the Tazwood Mental Health Center, Inc. (d/b/a the Tazwood Center for Wellness), are affiliates within the Central Illinois region of UnityPoint Health (UPH–Central Illinois). Additional UPH–Central Illinois affiliates/departments include, but are not limited to, The Methodist Medical Center of Illinois (Methodist), Proctor Hospital (Proctor), the Illinois Institute for Addiction Recovery (a department of Proctor), and Pekin Memorial Hospital.
