CMH Medical Clinic is a family practice group that provides care to all age ranges, newborn to geriatric, including: well-child visits, immunizations, routine pediatric care, shared maternity care, wound care, school and sports physicals, manipulations, and more. Our providers are happy to have the opportunity to care for you and your family!

Meet the Care Providers
Dr. Buchholz joined Community Memorial Hospital in July 2003. After graduating from North Fayette High School in 1992, he attended Wartburg College. Dr. Buchholz then went on to earn his MD degree from the University of Iowa College of Medicine in 2000. He spent the next 3 years in Cedar Rapids at a family practice residency program.
Dr. Buchholz was raised in Fayette and now resides in Sumner. He is married and has 3 daughters. He enjoys golfing, skiing, and doing other outside activities.
Dr. Shanna Elliott joined Community Memorial Hospital in 2018. After completing her undergraduate education at the University of Northern Iowa, Dr. Elliott attended Des Moines University to receive her DO and was a resident at Northeast Iowa Family Medicine Residency in Waterloo.
Dr. Elliott enjoys the science behind health care and is excited to use her knowledge to help individuals and their families. When she is not helping patients, Dr. Elliott spends her time traveling, bicycling and playing with her two dogs.
Spencer joined Community Memorial Hospital in April 2016. She received her Bachelor of Science in Nursing in 2012 from Allen College in Waterloo. She continued her advance education at Allen College where she also received her Master's Degree in Nursing in 2014.
Spencer grew up in Nashua and now resides near Readlyn. She is married and has two sons, Evan and Reid. Spencer enjoys anything that allows her time to spend with her family. She also enjoys reading and running.
Kayla joined Community Memorial Hospital in 2016. She received her Bachelor of Science in nursing in 2006 from Allen College in Waterloo. She continued her advanced education at Kaplan University, where she received her Master's Degree in Nursing in 2014.
Kayla grew up and continues to reside near Sumner. She is married and has three sons, Paxton, Brycen, and Kiptyn. Kayla enjoys spending time with family, her annual family vacation to Okoboji, riding bikes, and helping her husband on their beef farm.
