Tammy's True Story of Coordinated Care

Care Coordination Helps Equip With the Skills to Support and Manage Liver Disease and Daily Life 

Tammy Thompson was in the emergency room every few days because of severe pain from liver disease and food building up in her stomach. Her care coordinator at UnityPoint Health - Trinity in the Quad Cities,  set up a meeting with Tammy's family and her providers in order to find out what's important to Tammy and how they can help her function. Tammy wanted to spend more time at home with her daughter and less time in the hospital. As she improves psychologically and physiologically, she's able to do just that.
