How It Works
Your care will be coordinated by a UnityPoint Health physician, who will work along with our Post-Acute provider team at the Skilled Nursing Facility. They will work with you and the nursing facility to develop and use an individual care plan that is made just for you to help meet your individual needs. They will also communicate with your primary care physician so you have a smooth transition home.
Your UnityPoint Health Post-Acute provider team at the Skilled Nursing Facility will:
- Manage your day to day medical needs.
- Make sure the nursing facility continues your plan of care through communication and coordination with the SNF's team of physicians, nurses and therapists, who have different training and skills to help you recover.
- Teach you about your condition, including the medicines you need and the steps needed for recovery.
- Communicate with the physician who was caring for you when you were admitted to the SNF, as well as with any physician who will care for you after you leave the nursing facility.
It is important to know that you do not have to choose a UnityPoint Health Preferred Skilled Nursing Facility. Click here for a list of area facilities that are not part of our network. The places listed do not have the UnityPoint Health Skilled Nursing Facility Care. Therefore, they do not have the coordination of care program.
