Renewal Process:
How many hours do I need to relicense?
- EMTs need 60 hours in 4 years
- AEMTs/EMT-Is need 80 hours in 4 years
- Paramedics need 100 hours in 4 years
- Must have valid CPR, in addition AEMT/EMT-I and Paramedics need ACLS and PALS
What about the fee?
- Pay IDPH fee online, with the PIN sent to you.
What if I need a Volunteer Waiver?
Where do I send my information?
- Scan, fax or bring your hours to UnityPoint Health – Proctor EMS office. Please contact Mark McCandless for any question regarding your new license.
NREMT to Illinois State License:
NREMT to Illinois State License (for ICC students)
- A copy of the NREMT certification
- A copy of your CPR Card
- Fill this form out completely - and be sure to sign it.
- You will also need a cashier's check or money order for $45 made out to the Illinois Department of Public Health.
