Frequently Asked Questions about MyUnityPoint

What can I do with MyUnityPoint?

With MyUnityPoint, you can use the Internet to:

  • Review personal medical history such as your vitals and immunizations
  • Request appointments with your healthcare providers
  • See test results
  • Refill prescriptions
  • Read messages from your care team
  • Review your health summary
  • Securely message your care team

Who can use MyUnityPoint?

Anyone who is at least 18 years of age and is a patient of UnityPoint Health can register to use the service. 

How do I sign up?

You will be given a MyUnityPoint activation code during your hospital or clinic visit. This code will enable you to log in and create your own user name and password. Sign up by going to and entering your activation code. This activation code is good for 60 days in the event that you did not complete registration in the hospital.

Or, if you were not issued an activation code, you may request one by going to Chart.MyUnityPoint.organd selecting "sign up now" and clicking "contact us to register."  You will receive your activation code and information about how to complete registration within 3 business days.  

Can MyUnityPoint be used to view health information of my child or someone else I care for?

Yes, with proxy access you may:

  • Communicate securely with your health care team
  • Request an appointment with your healthcare providers
  • See test results
  • Renew prescriptions
  • Review your health summary

I forgot my password. What should I do?

You may click the "Forgot password" link on the sign-in page at to reset your password online. You may also contact our support team at, or call (877) 224-4430 Monday through Friday 8am-5pm CST to request a new, secure password. 

Can you send me a new activation code? I lost it, let it expire or did not receive it.

For your security, your activation code expires after 60 days and is no longer valid after the first time you use it. You may request a new activation code by selecting "sign up now" and clicking the "contact us to register."  Your request will be validated and returned to you within 3 business days.  Or, you may contact us at, or call (877) 224-4430 Monday through Friday 8am-5pm CST. After we verify your information, a new code will be sent to you via U.S. Mail. 

Can I access MyUnityPoint from my mobile device?

Yes. You can access a useful web-based version of MyUnityPoint from your mobile device. You can also download a free MyUnityPoint application from the iPhone App Store or Android Market by searching for MyUnityPoint. After download, just select MyUnityPoint from the app's phone book. 

What is your Privacy Policy?

MyUnityPoint is owned and operated by UnityPoint Health and is fully compliant with federal and state laws pertaining to your privacy. Your name and e-mail address will be treated with the same care and privacy given your health records and will never be sold or leased by UnityPoint Health.
