Medication reconciliation is a service provided to inpatients at GRMC. Upon discharge, patients receive a list of all their medications and over-the-counter supplements and medications recorded. This information is also stored electronically with the medical record.
Taking this list to all healthcare appointments can help bridge communication between providers. It is really important that healthcare providers know what patients are taking, how much they take and how often they take it. This includes vitamins, supplements and all non-prescription medications as well.
Everyone is encouraged to keep the information easily accessible. It is invaluable in an emergency healthcare situation to allow healthcare providers to know all your medications right away.
