Our commitment to provide you with quality care and service does not end when you are discharged from the hospital. For patients who would benefit from care in a Skilled Nursing Facility (SNF), we have developed a network of preferred providers. These providers are working closely with UnityPoint Health Hospitals and physicians to deliver coordinated care that leads to better patient outcomes.  

Your Skilled Nursing Facility will: 

  • Manage your day to day medical needs
  • Support the continuation of your plan of care through communication and coordination with the SNF's team of physicians, nurses and therapists, who have training and skills to help you recover
  • Teach you about your condition, including the medicines you need and the steps needed for recovery
  • Communicate with the physician who was caring for you when you were admitted to the SNF, as well as with any physician who will care for you after you leave the nursing facility
The Skilled Nursing facility network is one of the many care coordination services designed to provide quality care after your stay in the hospital. These are the regional places where your care may be coordinated by a UnityPoint Health physician, working with our Post-Acute provider team at the Skilled Nursing Facility.
