Mental Health Care and Counseling Services
UnityPoint Clinic is on the path to become a patient-centered medical home. A patient-centered medical home is a care model that has a team of health professionals addressing all of the unique health care needs of an individual. All care, including behavioral health, is then coordinated through a primary care provider. The model is really about addressing the whole person, their medical needs, as well as their behavioral health needs – together.
To provide this high level of coordinated care, UnityPoint Health and Hillcrest Family Services have created a partnership with the patient in mind. Hillcrest Family Services provides a licensed mental health counselor to visit each of UnityPoint Clinic's primary care locations. The therapist visits the Cascade, Peosta, Pennsylvania Avenue, and Women's Health clinic on a rotating basis so mental health care is more accessible for all of our patients.
Mental health counseling can cover anything from anxiety, stress, phobias, depression, ADHD, and more. If you are interested in counseling services, please talk with your primary care provider about what's best for you.
