UnityPoint Clinic is on the path to become a patient-centered medical home. A patient-centered medical home is a care model that has a team of health professionals addressing all of the unique health care needs of an individual. All care, including behavioral health, is then coordinated through a primary care provider. The model is really about addressing the whole person, their medical needs, as well as their behavioral health needs – together.
To provide this high level of coordinated care, UnityPoint Clinic Behavioral Health offers counseling and medication management services for individuals age 6 and older.
