UnityPoint Health - Eyerly Ball Community Mental Health Services is accredited by the Commission on Accreditation of Rehabilitation Facilities (CARF) serving individuals of all ages. At Eyerly Ball we offer a wide range of services to meet the needs of our clients and their families. Services include outpatient psychiatric evaluations and medication management, counseling and therapy, 24-hour emergency services and intensive community-based services for adults.
Our highly skilled team of experts will work closely with clients and their family to develop the best possible treatment and care plan for their individual needs. Each of our services was developed with the client and family in mind and we are dedicated to providing them with the care they need.
Outpatient therapy and medical services provide high quality, recovery focuses treatment including individual, couple and group counseling as well as psychiatric assessments and medication management in a confidential and supportive environment.
Residential Services strives to assist individuals who have severe and persistent mental illness in gaining or regaining the skills necessary to live independently within the community. Professional staff at the facilities provide quality care to our clients in a home setting and encourage least-restrictive living so residents can reach their goals and live a satisfying and rewarding life.
Community Based Services
Community Based Services offers services to people with serious mental health problems to assist them in living as independently as possible. Services available to these clients include home visits, problem solving, advocacy, monitoring of medication, symptom management, education and family support.
Care Coordination Services
Care Coordination is designated to help people successfully navigate the medical and mental health system. These services aim to help mental health patients have improved health outcomes by helping patients remove any barriers to medical care. Our Care Coordination teams listen to client concerns and then help them access the services that they need to get their concerns resolved.
Crisis Services provides teams of professionals that can provide on-site, face-to-face mental health services for an individual or family experiencing a mental health crisis. They can respond wherever the crisis is occurring – in an individual's home, the community or other locations where an individual lives, works, attends school or socializes. Within 24 hours of completion of the crisis assessment, a case manager will place a phone call to follow up with the individual. This case manager will be available to the client for up to 30 days following the crisis.
