UnityPoint Clinic Is Putting Patients First
About Patient-Centered Medical Home (PCMH)
At UnityPoint Clinic, we value putting patients first. We want your health care experience to be the best it can be. That's why we are redesigning the way we deliver your care by working together as a team and coordinating all the services necessary to benefit your overall health, whether it is in our clinics, our hospitals, or even in the comfort of your own home.
Your relationship with your primary care provider is very important to your health and well-being. At UnityPoint Clinic, we believe that your primary care experience is the heart of your health care. That's why we've adopted Patient-Centered Medical Home as our primary care model.
Goals of PCMH
UnityPoint Clinic is embarking on an exciting and important initiative to improve primary care for the people and communities we serve. Patient-Centered Medical Home will change the way we provide care for the better by:
- strengthening the patient-doctor relationship
- enhancing collaboration among the clinic team
- improving outcomes
PCMH is a primary care model designed to improve patient care through teamwork, access and technology. The medical home emphasizes the importance of an ongoing patient-doctor relationship for long-term health and utilizes a team-based approach to improve patient experience and engagement.
Your Provider Is Your Team Lead
The provider leads a clinic team that takes collective responsibility for patient care, education and support, and coordinates care with other providers or community services when necessary.
Medical homes provide a higher level of accessibility, whether through expanded hours, phone support or electronic communication. The PCMH model aims to provide more personalized, coordinated and effective care that improves outcomes and decreases cost.
