St. Luke's Transitional Care Center is a recuperative and supportive place to heal following an injury, surgery or serious illness. This facility is unique to our community. It's not a nursing home - it's a true transitional care center designed for short-term rehabilitation.
Building on our expertise with St. Luke's Living Centers, our professional approach to healing includes daily, individualized therapy sessions to help patients regain the strength, stability and confidence they need to function at their best. A dedicated team of providers, nurses, social workers, physical, occupational and speech therapists, and support staff help patients manage their health, so they can focus on getting better.
Families taking care of a loved one may use respite care for their family member to receive temporary relief from care giving while they travel or deal with other responsibilities.
For patients and families facing a terminal diagnosis, hospice care is available to provide comfort and supportive medical services. Working with UnityPoint Hospice and other hospice programs, our staff strives to offer a loving, peaceful and private environment for residents and families.
