Gary's True Story of Coordinated Care
Care Coordination Helps to Manage Chronic Illnesses and Take Back Control of Life
Gary Shaffer finally came to the emergency room at UnityPoint Health - Methodist in Peoria, after putting off getting help. His chronic lung and heart problems could be managed in a doctor's office, but he had no way of getting there. The emergency department worked with home health nurses to set up visits to Gary's home. As a result, Gary felt someone cared about him, and he took a more active role in his own health care. Now he's feeling better day-to-day, and his breathing has improved.
