If at any time you have a concern or are dissatisfied with your care at Unitypoint Health, please contact our Patient Advocate at 309-672-5529.  UnityPoint Health is committed to resolving care and safety issues in a timely manner and our Advocate program is dedicated to this purpose.  While we appreciate the opportunity to address your concern through our Advocate program first,  UnityPoint partners with regulatory agencies who may also be contacted with concerns as well,  including IDPH at 1-800-252-4343 (TTY: 1-800-574-0466 and Joint Commission at 1-800-994-6610 or email complaint@jointcommission.org.
