MyUnityPoint is a secure patient website that allows you to conveniently manage your personal health online. Because MyUnityPoint is web-based, you can log in at any time, any place.

What does MyUnityPoint allow me to do?

 Once your account is set up, you'll be able to: 

  • Communicate securely with your health care team
  • Request appointments
  • View lab results
  • Review your medical record
  • Renew prescriptions

In the future, you'll also be able to make payments.

Who can participate?

All UnityPoint patients 18 and older are invited to use the MyUnityPoint website.

How do I sign up? 

Follow these easy steps:

  1. Click on the "Get Started" link and enter your name, birth date, phone number and email address.

    Note: Email addresses with the following domains tend to block our registration messages:,,,,,, and anything ending in .edu , .us, and .org.  Also you may not want to use an email address that you share with another person unless you are willing to share your medical information with that person."

  2. Within a day or two, you will receive an email message from FollowMyHealth (what's this?) with a link to register. After clicking on the link, you will be asked for an invitation code for security purposes. Your invitation code is the last four digits of your social security number.

How do I set-up and manage an account for my elderly parent or someone else in my care?

Follow these easy steps:

  1. Click on the "Get Started - Proxy Access" link, review the Guidelines for Requesting Proxy Access, and complete the Proxy Access Form
  2. After about 10 days, you will receive a notification email message from FollowMyHealth (what's this?) with instructions and a link to complete a final step to establish the account.
