Who can participate?

All UnityPoint patients 18 and older are invited to use the MyUnityPoint website.

How do I sign up?

Follow these easy steps:

  1. Click on the "Get Started" link and enter your name, birth date, phone number and email address.

Note: Email addresses with the following domains tend to block our registration messages: AOL.com, Q.com, iowatelcom.com, netins.net, imonmail.com, rr.com, and anything ending in .edu , .us, and .org.  Also you may not want to use an email address that you share with another person unless you are willing to share your medical information with that person."
