For Patients

Welcome to UnityPoint Health – Meriter

Thank you for choosing UnityPoint Health – Meriter. Our healthcare professionals have dedicated their lives to your good health, and they will make every effort to ensure that your stay is as pleasant as possible.

We encourage you to take an active part in your health care by asking questions and expressing concerns if you don't understand what you have been told, learning about your diagnosis and the medical tests and treatment plan you have been prescribed, and knowing the medications you are taking.

Patient and Family Feedback

If you or a loved one recently received care within the Meriter system, we would appreciate your feedback. Your comments will help us learn what you liked and what we can do to better meet the needs of our patients and the community. Provide feedback.

My Meriter Story

UnityPoint Health - Meriter feels privileged to be able to care for patients, families and loved ones during times of happiness and sorrow. We believe a community grows stronger and more compassionate by sharing in others' experiences.

Through My Meriter Story, you can share your personal account on how those at Meriter have made an impression on you (as a patient, a visitor or an employee), and read what others have written. You'll also learn more about the physicians, nurses and other Meriter staff members who provided exceptional care.

Have a special story about your Meriter experience? We'd be honored if you would share it with us. Share your My Meriter Story.

Patient Rights and Responsibilities

As a patient, you have the right to voice complaints regarding the quality of care and services you receive, and are assured that a complaint will in no way compromise your access to care. At UnityPoint Health - Meriter, patient representatives are available for any problems, concerns or complaints that hospital patients or their families may have while at Meriter. A representative will work on your behalf to address these issues.

To reach the Patient Representatives, call (608) 417-6462 (extension 7-6462 from inside the hospital). When the Patient Representatives are away from the hospital, the Nursing Administrative Coordinator will respond to any concerns that may arise. You can also talk to your physician, nurse or nurse manager at any time.
