From pediatric to senior home care, the UnityPoint at Home team offers a wide range of services with the goal of providing the necessary support and health equipment necessary to promote independent living.

Pediatric Home Care

Our Pediatric Home Care services are tailored to meet the unique needs of the young patients we serve. The pediatric team is committed to providing quality care coupled with compassion to create a comfortable environment. In addition to medical support, our team will work with each family to help them learn how to care for their special needs child. Whether you have a medically fragile infant, child or adolescent, our skilled team will work with you and your pediatrician to develop a custom coordinated care plan that will allow your child to have a safe home environment. Services provided by our Pediatric Home Care program include:

  • An individualized assessment of each young patient’s needs, created with the family situation in mind
  • Assistance with daily activities and therapy to help the child learn to do these activities independently
  • On-site nurses and licensed practical nurses that provide care according to the child’s individual needs
  • Equipment to provide care for pediatric patients from the comfort of their home, including ventilators, IV and TPN, trachs, g-tubes, central lines, monitoring equipment of all types, wheelchairs and nebulizers
  • Pediatric speech, respiratory, occupational and physical therapy

Adult & Senior Home Care

UnityPoint at Home brings medical services to your home, allowing you to remain comfortable following an injury, surgery or illness. The patients our trusted Adult & Senior Home Care team works with includes:

  • Individuals with serious illnesses or life-limiting conditions
  • Individuals with a chronic illness such as diabetes, lung disease or heart disease
  • Seniors or adults with cognitive or physical disabilities
  • Post-surgical patients needing additional wound care and medication management
  • Patients needing physical therapy following a joint replacement
  • An individual transitioning home after an extended stay at the hospital

Palliative Care

Regardless of age or stage of life, Palliative Care is available from UnityPoint at Home to provide comprehensive medical care for patients dealing with a serious illness. Our Palliative Care team focuses on pain and stress relief in addition to alleviating symptoms associated with the illness. Patients who benefit from palliative care are:

  • Individuals of any age with cancer, kidney disease, lung disease, heart failure, dementia or other serious or life-threatening conditions
  • Individuals experiencing symptoms such as pain, fatigue, shortness of breath, loss of appetite, depression or other symptoms that compromise their quality of life
  • Individuals who have been hospitalized or had multiple emergency room visits 
  • Individuals wanting to discuss goals of care or advanced care planning options

Palliative Care is available to patients of all ages and at any stage of their disease. It can be short- or long-term care and can be received simultaneously with curative treatments.

Hospice Care

Hospice Care is available to patients with a life-limiting illness that is no longer responding to treatment. Hospice Care is provided for patients who are expected to be within the last six months of life, and the care does not prolong life or hasten death, but instead focuses on making the patient as comfortable as possible during the final stages of life. Hospice care at UnityPoint is coordinated between a medical director, physician, nurse and others who all work together to provide care to relieve suffering and improve the quality of life. Services provided include:

  • Home and inpatient care
  • Symptom and pain control
  • Respite care
  • Family conferences
  • Spiritual care
  • Bereavement support

Home Medical Equipment Services

In addition to in-home medical services, UnityPoint at Home offers Home Medical Equipment Services that provide a comprehensive selection of home medical equipment, products, services and related education. Our dedicated staff is available 24 hours a day to help you remain safe and healthy in your own home. Services available include:

  • A personalized assessment to create a care plan that is continually updated to ensure the best patient outcome
  • Training to ensure each patient understands their plan, how to use each piece of equipment and who to contact in case of an emergency
  • On-call service technicians to service (repair and/or maintain) all equipment
  • On-site delivery and setup
  • Insurance and Medicare billing
  • Home medical equipment centers, conveniently located, staffed with specially-trained technicians to work with patients individual needs for purchased or rental equipment
