At GCMH Long Term Care, the focus is on providing the highest quality of care in a safe and comfortable environment. Professional and compassionate nursing staff is available 24/7 to offer assistance with medical and personal care for those who are no longer able to carry on the normal activities of daily living on their own.  

Staff works together to create an individualized plan of care for each resident based on his or her specific needs, requests, and preferences. Our social worker is available to assist residents and families as they transition into this new setting. The social worker supports residents with adjustments to changes in health, insurance issues, and helps connect residents to community resources.

Long Term Care Cottages - Open Spring 2018

The Grundy County Memorial Hospital has partnered with Western Home Communities of Cedar Falls to offer management services to the GCMH Long Term Care.  It's exciting to share that Western Home Communities is building three Long Term Care Cottages near the hospital campus to accommodate up to 55 residents.  Current hospital Long Term Care residents will have the first opportunity to move to the modern Cottages, called Creekside, when they open for nursing care in the spring of 2018.  If you are interested in nursing care at Creekside for yourself or a family member, contact Kim Schilling, administrator, or Kristi Barnett, social worker.
