Health Insurance

Employees may have coverage for vision, chiropractic, pharmacy, and mental health/substance abuse benefits.

Employee premiums are taken each pay period and are deducted on a pre-tax basis allowing employees to lower taxable income and increase take home pay.

First day of the month following date of hire or date of eligibility.

 

Regular full-time, part-time and weekend package employees are eligible.

Regular full-time and part-time employees(UnityPoint Health - Des Moines and Employee);

 

Weekend Package employees pay full premiums (employer + employee premiums)

 

Prescription Drug Program

Prescription drug coverage through participating pharmacies. Co-payment for prescriptions are determined by: Generic: $10, Formulary Brand: $30, and Non-Formulary: $60.

First day of the month following date of hire or date of eligibility.

Regular full-time, part-time and weekend package employees are eligible.

Dental Plan

Services include two free dental cleanings per year.

First day of the month following date of hire or date of eligibility.

Vision Plan(hardware only)

Employees may choose to participate in a voluntary vision plan that offers coverage for eyeglass frames, lenses and/or contact lenses.

Paid Time Off (PTO)

PTO combines vacation, holidays, sick and personal days into a flexible plan up to a maximum of 200 hours per year for non-exempt employees and 240 hours for exempt employees.

Accrual rates increase after five years of employment is completed.

Regular full-time and part-time employees. After 6 months of employment.

Paid Time Off Purchase program

Each Spring and Fall, UnityPoint Health - Des Moines allows eligible employees to purchase a pre-defined amount of PTO, based on years of service.

Short Term Disability (STD)

This plan will cover a maximum of 26 weeks of disability and pay at 60% of the employee's average weekly wages.

Coverage begins on the 8th calendar day of being away from work due to a non-work related injury or illness.

Any regular full-time, part-time or part-time accruing employee averaging at least 16 hours per pay period.

After the employee has worked 1,000 hours at UnityPoint Health - Des Moines.

Long Term Disability (LTD)

Coverage level of 60% of your income.

First day of the month following date of hire or date of eligibility. 

Regular full-time employees that work at least 64 hours a pay period.

Life Insurance

Core life for a regular full-time employee is 1 x base annual salary rounded up to nearest $1,000.

Core life for a regular part-time employee is $10,000.

Eligible employees are given the option to carry additional life insurance on themselves, spouses, and their dependents.

401(k) Retirement Plan

Pre-tax retirement savings plan. UnityPoint Health - Des Moines-Des Moines will automatically contribute 2% of your pay into your account each year.

You may contribute up to $16,500 in 2009 and UnityPoint Health - Des Moines will match 50% of your contribution up to 6% of your pay.
