Preparing for an Appointment

Scheduling

When you call to schedule an appointment with one of the clinics at UnityPoint Health, it is important to inform them of your reason for the visit, as this will help us find the best physician for your needs. If you need to reschedule or cancel an appointment, please notify us at least 24 hours in advance.

How to Prepare

When coming in for an appointment, it is helpful to have some information ready for the clinic and the physician. It will help your visit go more efficiently and also help you remember any details that may be important for the physician to know in order to best assess your health and plan your care:

  • Symptoms, even if they may seem unrelated
  • Medication or vitamins you are currently taking
  • Questions you may have for your physician
  • Insurance card and photo ID

In order to stay on time with all the appointments for a physician, we request that patients arrive 10 minutes before their scheduled appointment to allow for enough time to fill out any necessary paperwork for registration.

Medical Record Release

Your medical records are confidential and require written consent by the patient or guardian in order to be released from any clinic location. Processing requests for medical records could take up to two weeks. Please request all copies in advance so that we can meet your needs in a timely manner.
