UnityPoint at Home Home Care brings one-on-one, professional health care services to you in your home where you're surrounded by the people who care for you best. We work to restore your independence so you may remain in your home for as long as possible.

We care for people of all ages including children, adults, new mothers and seniors. Our goal is to help you improve your health and live better. At the same time, we educate you on the tools you and your family will need to get back to your day-to-day routine.

Discuss home health care options with your doctor if you think Home Care services will help you stay in your home and maintain your independence. Home Health Care Services

Home health care visits help individuals and families develop and maintain appropriate health practices for
the benefit of the patient. Services are available to anyone within a 50 mile radius of Cedar Rapids. Our home health care services include but are not limited to:

  • Skilled Nurses: Assess your health, change dressings, administer medication, educate on cares and more

  • Physical Therapists: Develop in-home rehabilitation and strengthening programs to maintain and restore your movement

  • Occupational Therapists: Work to improve your ability to perform everyday activities

  • Speech Therapists: Treat hearing, communication and swallowing disorders
  • Certified Nursing Assistants: Assist you with personal cares, such as bathing and grooming
  • Medical Social Workers: Offer services to you and your family to help you deal with the social and emotional aspects of your illness
  • Telehealth Monitors
  • Maternal & Child Health: Children’s health services include physical exams, immunizations, wellness checks and more. They focus on disease prevention from birth through age 21. Maternal health services include prenatal education, social and nutrition assessment and counseling for eligible pregnant women.
  • Senior Health Clinics: Provided at housing complexes and meal sites, senior clinics strive to help chronically ill seniors improve the way they care for their own health. Periodic checks monitor health information and prevent medical crises. Attendees receive information on resources in the community and referrals to appropriate resources for assistance. Clinics are free to all.
